Assessment Tasks

Copy and paste from the following data to produce each assessment task. Write these in plain English and spell out how, when and where the task is to be carried out, under what conditions, and what resources are needed. Include guidelines about how well the candidate has to perform a task for it to be judged satisfactory.
Copy and paste from the following performance criteria to create an observation checklist for each task. When you have finished writing your assessment tool every one of these must have been addressed, preferably several times in a variety of contexts. To ensure this occurs download the assessment matrix for the unit; enter each assessment task as a column header and place check marks against each performance criteria that task addresses.

Assist carer and family to determine assistive technologies that support the person and themselves and confirm understanding of its use. 
Assess risks of change to the care relationship including potential physical and psychological harm to carer, family and the person. 
Use identified strategies to address risks according to organisational policies and procedures and legal and ethical considerations. 
Support the person, carer and family to determine and use strategies that maximise positive aspects of change and transition. 
Support carer and family to maintain ongoing support and involvement in the life of the person. 
Respect the confidentiality and privacy of the carer and family, as well as the person with support needs. 
Recognise and respond to the need for services required by the carer and family to support the care relationship with the person. 
Recognise and respond to issues that may impact on the physical and emotional health and wellbeing of the carer and family. 
Provide carer and family with information about support services and how to access.
